Clubalfa Member Registration Process

1. Fill up an application Form (see sample application form complete with email address) by the applicant to be submitted to a Janssen Product Specialist.
2. Product Specialist endorses his name and signature in the member application form.
3. Product Specialist forwards endorsed application forms to Len Temperante for submission to clubalfa.com.ph/Xorand.
4. clubalfa.com.ph generates and sends automatically a welcome letter for new members informing
them too of their user id and password. User ID will be the email id of the member. Password will be random generated.

1. Eprex Patient
2. With email address

Clubalfa Doctor Registration Process

2. Product Specialist endorses his name and signature in the member application form.
3. Product Specialist forwards endorsed application forms to Len Temperante for submission to clubalfa.com.ph/Xorand.
4. clubalfa.com.ph generates and sends automatically a welcome letter for new members informing them too of their user id and password. User ID will be the email id of the member. Password will be random generated.

Clubalfa Doctor Qualifications :

1. Doctor must be a Nephrologist or an Oncologist.
2. With email address

Online Applications Process :

1. Subscribers of www.clubalfa.com.ph will only be allowed to accomplish and print application forms.
2. The printed and signed application form must thereafter be personally submitted to the Product Specialist for endorsement.

First Time Access :

The subscriber can use the user id and password emailed by clubalfa.com.ph. The password will have to be immediately changed however.
The subscriber is also encouraged to update his profile particularly the 'Forgot Password?' field and the answer to it for future incidents when the user forgets his own password. If the user makes the correct answer to the 'Forgot Password?' in the future, he will allowed to change his forgotten password into a new one.
